New data on disability-adjusted life years due to diabetes from the national burden of disease study BURDEN 2020

Diabetes mellitus is a chronic metabolic disease that is associated with an increased risk of concomitant and secondary diseases (complications) as well as increased mortality. As part of the BURDEN 2020 study at the Robert Koch Institute, the burden of disease due to morbidity and mortality was calculated for various diseases. The results for diabetes were included in the diabetes surveillance.

Years of life lost (YLL)
Diabetes can result in years of life lost due to premature mortality. In 2017, diabetes was responsible for 267,600 of the years of life lost due to death in the population of Germany. The vast majority of the years of life lost due to death are attributable to type 2 diabetes. Diabetes is thus one of the most important causes of death among non-communicable diseases.

Years lived with disability (YLD)
Diabetes can be associated with functional and quality of life limitations due to serious complications, resulting in a loss of symptom-free years of life. The number of years of life lost due to health disabilities allows conclusions to be drawn about the morbidity-related burden of disease. In 2017, diabetes led to a loss of 526,823 life years due to health disabilities in the population of Germany. Type 2 diabetes accounts for the vast majority of the morbidity-related burden of disease. In the age groups under 30 years, however, type 1 diabetes is in the foreground.

Disability-adjusted life years (DALY)
Summary measures are used to calculate the contribution of disease and death to the health of the population as a whole. A commonly used measure here is DALY, which is made up of the YLL and YLD. In 2017, the burden of disease from diabetes in the population of Germany was 794,940 DALY. While in the case of type 1 diabetes more life years were attributable to death (YLL), in the case of type 2 diabetes the burden due to health disabilities (YLD) predominates.

Furthermore, the time series of some indicators were continued for the year 2020 and results stratified by social deprivation for two indicators (Diabetes-related amputations, Ambulatory care-sensitive hospitalisations) were added:
